Medicare Savings Programs: QMB, SLMB, and QI Explained

Medicare Savings Programs are state-administered programs that can pay some or all of your Medicare Part A and Part B costs -- and one of them comes with real protection against being billed at all.

Medicare Savings Programs (MSPs) are administered through your state's Medicaid program and can help pay Medicare premiums and, for the most comprehensive tier, cost-sharing too. There are a few different tiers -- QMB, SLMB, and QI -- and which one you might qualify for depends on where your income falls, with real differences in what each one actually covers.

QMB is the most comprehensive tier

The Qualified Medicare Beneficiary (QMB) program can pay your Part A and Part B premiums, deductibles, coinsurance, and copays. If you have QMB, Medicare providers are generally prohibited from billing you at all for Medicare-covered services -- a real, federally enforced protection, not just a discount.

SLMB and QI help with the Part B premium only

Specified Low-Income Medicare Beneficiary (SLMB) and Qualifying Individual (QI) don't cover deductibles or coinsurance the way QMB does -- they specifically pay your Medicare Part B premium, and are available at somewhat higher income levels than QMB.

QI has limited annual funding and generally requires reapplying every year

Unlike the other tiers, QI is funded through a limited annual allocation and is generally granted on a first-come, first-served basis each year -- worth applying early in the year if this is the tier that fits your income.

Qualifying for any Medicare Savings Program generally means automatic Extra Help too

If you qualify for QMB, SLMB, or QI, you're generally automatically enrolled in Extra Help for your prescription drug costs as well -- one application can address both Part B and Part D costs.
