- Whether your specialist and your hospital are in network.
- Whether each of your medicines is on the plan's drug list, and what tier it sits in.
- The out-of-pocket maximum, since you are likely to reach it.
- Whether you need referrals or approvals for regular care.
Why the monthly premium is not the whole story
If you expect to use care regularly, a plan with a higher premium and lower cost sharing can cost less in total than a cheaper plan with a high deductible. Total yearly cost is the number to compare.

No. ACA-compliant plans cannot exclude coverage of pre-existing conditions.
